When booking an appointment you will be asked to complete a short medical and consent form this is to enable me to tailor your treatment to your individual needs. This personal information will be securely stored either in an electronic or hard format. Once this information is no longer required it will be securely destroyed.
Your personal information will never be shared with a third party.
You have the right to make a ‘subject access request’ should you wish to see the personal data held about you. You can do this by emailing your request to anna@reflexandunwind.co.uk.
